According to a complaint filed by his son, Mohammed Safwan, the incident occurred on June 28 while Ismail was erecting a shed in front of a shop. Two iron pipes had already been fixed in place, and while lifting another iron pipe measuring around 20 feet in length to remove sand from inside it, the pipe accidentally touched an overhead electricity line passing in front of the shop.

As a result, Ismail suffered a severe electric shock and collapsed on the spot.

He was immediately rushed by his son to a private hospital at Padil in Mangaluru, where doctors declared him dead later that night despite efforts to save him.
